Problem with modular page form

Hello Grav

I have a form in a modular page that for some reason stop working after updating.

The form.yaml has been checked briefly by @Flaviocopes and is working well on a simple page but on a modular one, the same form.yaml do not work anymore: whatever original page or blank modular page) it does not output.

I have been digging around everywhere since 4 days (see previous post below), I can't get it back.

please help

it's was the asset and maintenance plugin!

*see bottom of this page > If the form fields do not appear if you have installed the Assets plugin disable / uninstall it. There's a known issue with it breaking modular forms will be soon fixed
